Is it my responsibility to keep track of my credit hours?
Yes. It is the applicant’s responsibility to keep evidence of his/her CDE activities for four years following license renewal in the event of an audit by the State Board of Dentistry. Evidence of course attendance and completion may take the form of vouchers, certificates or letters from approved sponsors, sponsor-generated printouts, or a certified transcription of the courses taken from an accredited college or university.
